To be considered for appointment with the Committee, all youth, including returning youth, must complete an application. All applications for representatives will be administered and reviewed, and only those selected to participate will be contacted. All Committee members will receive community volunteer hours throughout their term.
*Members commit to leadership term (September-May) and receive documented civic leadership hours.

- Availability confirmation
- Parent/guardian consent
Personal information being collected in this survey is collected under the authority of the Municipal Act, 2001, S.O. 2001, c. 25 and disclosure of responses will be governed by the Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. M.56. Your personal information is confidential and will only be reported in a de-identified, aggregate manner to staff in regards to the application to be part of the Burlington Youth Student Council.
